What is Intertirgo?

Intertrigo is a common inflammatory skin condition that occurs due to friction between skin surfaces within skin folds, where moisture is trapped by poor air circulation. This condition can occur on any area of the body where two skin surfaces are in close contact, such as the armpits, under the breasts, the navel and the groin area. Intertrigo is strongly associated with obesity.

Intertrigo initially presents as a specular erythema in the skin folds, often accompanied by itching, stinging and burning. It may progress to more severe inflammation with erosion, oozing, exudation and maceration, and secondary infection is a frequent complication. In this regard, the combination of warm, moist and damaged skin provides ideal conditions for micro-organisms to reproduce.

As the stratum corneum macerates due to hyperhydration, friction intensifies and further weakens and damages the epidermal tissue. The condition can progress to severe inflammation and skin breakdown. This erosion of the epidermal barrier can create an entry point for micro-organisms causing secondary infections.

The prevalence of intertrigo varies from 6% in hospitalised patients to 17% in nursing home patients and 20% in home care patients. Intertrigo can affect individuals of any age, with babies and infants being especially prone to develop intertrigo in the neck folds, due to their short necks, flexed postures and drooling. In adults, several factors may contribute to the development of intertrigo, such as obesity, diabetes mellitus, immobility/bedding, urinary and/or faecal incontinence, malnutrition, hyperhidrosis, use of nappies, exposure to hot and humid environments and wearing occlusive clothing.

Intertrigo can present clinically in a range from mild erythema to extensive skin breakdown. The damage observed is usually reflected on both sides of the skin fold. Intertrigo causes itching, stinging, burning, prickling and painful sensations and may also produce odour. As a result, the quality of life of people with intertrigo can be significantly affected.

Prevention and treatment should start with a thorough assessment of all skin folds. It is suggested that early identification of skin damage is essential to avoid further deterioration and poor outcomes. If not effectively treated, any initial secondary infection in intertrigo can easily progress to more serious soft tissue infections, such as cellulitis, or even lead to sepsis.
